
Health Practitioners Registration
PROVISIONAL REGISTRATION
This is a register for all locally trained Health Practitioners. The Provisional Registration period is one year for both diploma and degree holders except for Medical Doctors who undergo internship for one year six months. All Practitioners on this register work under supervision of an approved supervisor. Upon successful completion of this registration period a Practitioner is required to apply for Full Registration after being recommended by their supervisor and head of institution.

Certified copies of academic transcripts and professional qualification from a training institution offering recognized and approved training programs by the Council

Proof of Student Indexing (Index Number)

Certified copies of Grade Twelve Certificate/GCE or statement of Results

Medical examination report from a government health facility in Zambia

One passport size photograph (white background-observe formal dressing)

Certified copy of NRC or Passport for non-Zambians.

Completed privilege to supervise form for those seeking employment in a registered private health facility.

Certified declaration by the Commissioner of Oaths/Notary Public

Provisional registration is valid for a maximum period of one year (12 months) and can be extended for another period of one year if a practitioner does not meet the minimum requirements for full registration.

LIMITED REGISTRATION
This is a register for all Health Practitioners who qualified in another country and have been invited by a registered Health facility in Zambia to provide health care for a period not exceeding six months. All Practitioners on this register work under supervision of an approved supervisor.

Requirements for Limited Registration of health practitioners in Zambia are as follows:

Certified declaration by the Commissioner of oaths/Notary Public

Submit the following:

Letter of invitation/offer of employment from prospective employer in Zambia

Proof of Registration from the Country of Origin or Country the practitioner Last Practiced

Certificate of Status (Good standing) from country the practitioner last Practiced
-
Certificate of competence in English from British Council if applicant is from non English speaking country
-
Photocopies of professional primary qualifications i.e. certificates, diplomas or degrees certified by the Commissioner of oaths/Notary Public or the embassy representing his/her country in Zambia.
-
Completed Privilege-to-Supervise-Form form for those seeking employment in a registered private health facility.
-
One passport size photograph (white background-observe formal dressing).

Limited registration is valid for a maximum period of six (6) months and the application should be submitted four (04) weeks before the visit.

TEMPORARY REGISTRATION - TRAINED OUTSIDE ZAMBIA
This is a register for foreign trained Health Practitioners. The Temporary registration period is two years for all Health Practitioners. All Practitioners on this register work under supervision of an approved supervisor. Upon successful completion of this period a Practitioner is required to apply for full registration after being assessed by the supervisor and recommended their head of institution.

Requirements for Temporary Registration of health practitioners trained from outside Zambia are as follows:
1. Completed HPCZ Temporary registration application form
2. Certified declaration by the Commissioner of oaths/Notary Public

Certificate of Status (Good Standing) from country the practitioner last practiced

Certificate of competence in English from the British Council-Zambia if applicant is from non English speaking country.

Temporary registration is valid for two years.

FULL REGISTRATION
This is a register for all Health Practitioners who have successfully completed their provisional or temporary registration period. If a Practitioner’s recommendations are not satisfactory, the Council extends the registration period from up to twelve (12) months to enable the Practitioner improve to the required standard. Practitioners on this register are eligible to supervise Practitioners on Provisional, Temporary and Limited Registers. Practitioners on Full Register can work with minimum supervision.

Requirements for Full Registration of health practitioners in Zambia are as follows.

Completed, signed and stamped assessment form by approved supervisor i.e. supervisor who is fully registered by HPCZ and is of same profession as applicant or is a medical doctor.

Recommendation letter from the head of institution

Proof of completion of internship for junior resident medical officers and medical licentiates (attach signed rotations form)
-
Medical examination Report from a government health facility in Zambia
-
One passport size photograph (white background-observe formal dressing)
-
Certified copy of Professional Qualification
-
A copy of previous HPCZ Registration certificate (Provisional/Temporary)
-
Must have been working for a period not less than 12 Months(provisional) and 24 Months (Temporary)

Health Practitioner who holds a Provisional or Temporary Registration Certificate and is eligible for full Registration must ensure that they apply for registration a Month before expiry date.

SPECIALIST REGISTRATION
This is a register for all Practitioners who hold a post graduate degree and a post graduate qualification in a field relevant to their primary qualification. A Practitioner can only apply for Specialist Registration if he or she is on the full register.

Recommendation letters from the Head of the Institution and from a practitioner of the same specialty

Copies of Full Registration Certificate and a current Annual Practicing Certificate

A detailed curriculum vitae

One passport size photograph with White background (Observe formal dress).

Certified photocopy of the National Registration Card or Passport for non-Zambians.

Certified photocopies of transcripts of primary and post graduate professional qualifications (i.e. Degrees, Masters
-
Degrees or Doctorate) recognized by the Health Professions Council of Zambia.
-
Medical Examination Report from any public health facility.
-
Foreign qualifications to be verified by Zambia Qualifications Authority (ZQA) and Education Commission for Foreign Medical Graduates (ECFMG).

CERTIFICATE OF STATUS
A Certificate Of Status (Certificate of good standing) is issued to any Health Practitioner who needs registration with another regulatory authority in another country or /training institution. Only Practitioners who have been practicing for a minimum period of one year on the full register and have no disciplinary proceedings against them qualify for issuance of this certificate. This certificate is valid for six (6) months.

Requirements for Certificate of Status (Good Standing) are as follows;

Completed HPCZ application Form 5 and signed by the head of institution (or the last employer for those practitioners who have left the country).

Proof of payment (non-refundable fee)

Submit requirements for registration from.

offer of employment or

a training institution or other supporting documents from institutions where the certificate is required.

Submit a letter stating the reasons why you are applying for a certificate of good standing.
Note:
-
Practitioners with cases in progress or pending with the disciplinary committee of the Health Professions Council of Zambia are not entitled to a certificate of good standing.
-
Practitioners in annual fees arrears will not be issued with a certificate of good standing.
-
Practitioners with less than twelve months period on full register are not eligible for a certificate of good standing.

Practitioners to provide professional reference where the head of institution is not a registered practitioner under HPCZ (Professional body/Certified HPCZ supervisor)

ANNUAL PRACTICING CERTIFICATE
It is a duty of every practitioner to ensure that annual practice certificates are renewed before the first of January every year. All practitioners without a practicing license of the particular year will be regarded as practicing illegal and maybe be prosecuted.
